DOHE: The Database of Human Errors

What Happened:
I fell while skiing last Friday and lost my cell phone. Didn't realize it till the drive home.
Root Cause:
Did not secure the phone well. Should have identified this as a high risk outcome since I was, after all, on the steep side of a mountain covered by snow. Likelyhood of finding it would be very low.
How to prevent it from happening again:
Be extra careful when traveling in awkward places at high velocities that you might drop something valuable. Don't put it in unsecure places, like an unzipped coat pocket.
